Within the cell, vitamin K participates in a cyclic process. The vitamin undergoes electron reduction to a reduced form called vitamin K hydroquinone (quinol), catalyzed by the enzyme vitamin K epoxide reductase (VKOR). Another enzyme then oxidizes vitamin K hydroquinone to allow carboxylation of Glu to Gla; this enzyme is called gamma-glutamyl carboxylase or the vitamin K–dependent carboxylase. The carboxylation reaction only proceeds if the carboxylase enzyme is able to oxidize vitamin K hydroquinone to vitamin K epoxide at the same time. The carboxylation and epoxidation reactions are said to be coupled. Vitamin K epoxide is then restored to vitamin K by VKOR.
== Clinical significance == Mutations in SPINK1 has been associated with hereditary pancreatitis and tropical pancreatitis. Trypsinogen is normally created and stored an inactive zymogen of trypsin in the pancreas, but occasionally will autoactivate itself. PSTI serves to cleave prematurely activated trypsin to prevent the enzyme from causing cellular damage to the organ. Without the function of PSTI, the pancreas is subject to repeated episodes of damage. It has also been associated with prostate cancer.

The Socialist Republic of Bosnia and Herzegovina (SR Bosnia and Herzegovina; SR BiH), commonly referred to as Socialist Bosnia or simply Bosnia, was one of the six constituent republics forming the Socialist Federal Republic of Yugoslavia. It was a predecessor of the modern-day Bosnia and Herzegovina, existing between 1945 and 1992, under a number of different formal names, including Democratic Bosnia and Herzegovina (1943–1945), Federal State of Bosnia and Herzegovina (1945–1946), and People's Republic of Bosnia and Herzegovina (1946–1963). It was administered under strict terms of sanctioned consociationalism, known locally as "ethnic key" or "national key" (Serbo-Croatian: etnički/nacionalni ključ), based on the balance of political representation of 3 largest ethnic groups (Bosnian Muslims, Croats and Serbs). Sarajevo served as the capital city throughout its existence and remained the capital following independence. The Socialist Republic was dissolved in 1990 when it abandoned its socialist institutions and adopted liberal ones, as the Republic of Bosnia and Herzegovina which declared independence from Yugoslavia in 1992. == Causes == Adverse drug reactions are classified as type A (intrinsic or pharmacological) or type B (idiosyncratic). Type A drug reaction accounts for 80% of all toxicities. Drugs or toxins that have a pharmacological (type A) hepatotoxicity are those that have predictable dose-response curves (higher concentrations cause more liver damage) and well characterized mechanisms of toxicity, such as directly damaging liver tissue or blocking a metabolic process. As in the case of paracetamol overdose, this type of injury occurs shortly after some threshold for toxicity is reached. Carbon tetrachloride is commonly used to induce acute type A liver injury in animal models. Idiosyncratic (type B) injury occurs without warning, when agents cause non-predictable hepatotoxicity in susceptible individuals, which is not related to dose and has a variable latency period. This type of injury does not have a clear dose-response nor temporal relationship, and most often does not have predictive models. Idiosyncratic hepatotoxicity has led to the withdrawal of several drugs from market even after rigorous clinical testing as part of the FDA approval process; Troglitazone (Rezulin) and trovafloxacin (Trovan) are two prime examples of idiosyncratic hepatotoxins pulled from market. == Treatment == Treatment of tea and toast syndrome is centered primarily around resolving hyponatremia. Treatment choice depends on the type of hyponatremia. Traditional treatment for hyponatremia depends on the volume load in the person. For those who are euvolemic (normal body volume load), fluid intake should be restricted. In those that are hypovolemic (low body volume load), give isotonic saline. In those that are hypervolemic (high body volume load), diuresis should be induced. Elderly patients may present in any of these volume states. However, "tea and toast" syndrome patients typically present euvolemic hyponatremia since their hyponatremia is caused by low solute intake. These patients must receive proper nutrition that is higher in protein and electrolyte intake. The Health and Medicine Division of the National Academies of Science, Engineering, and Medicine recommends that older adults improve their diet by consuming more fish, low-fat dairy products, and whole grains (such as fortified breakfast cereals). Older adults can also improve their diet by snacking on nuts and legumes as well as ensuring that they include fruit and vegetables at the center of their diet.
=== Effectiveness evaluation === A 2013 article published in The Lancet reported that the MenAfriVac vaccination campaign in Chad reduced meningitis incidence by 94%. In three regions of Chad, approximately 1.8 million people from 1 to 29 years old received a single dose of the vaccine in December 2011. Vaccinating 70% of the population in that age group is enough to create "herd immunity". During the 2012 meningitis season no cases of the meningococcus sub-type serogroup A caused disease in places where mass vaccination took place. Carriers of serogroup A were found to decrease by more than 97% post-vaccination. Surveillance is needed to continue for several more years to establish the length of effective period of the vaccine and whether other meningococci serogroups may surge to replace serogroup A. In November 2015, a special collection of 29 articles was published in the journal Clinical Infectious Diseases—with guest editors from Public Health England and the former Meningitis Vaccine Project about the steps taken for the development, introduction, and evaluation of MenAfriVac. Immunization with MenAfriVac has led to the control and near elimination of deadly meningitis A disease in the African "meningitis belt". In 2013, only four laboratory-confirmed cases of meningitis A were reported by the 26 countries in the meningitis belt. But scientists warned that unless countries within the belt incorporate the meningitis A vaccine in routine immunization schedules for infants, there is a risk that the disease could rebound in 15 years' time.
I am myself a Christian, a member of a community that preserves an ancient heritage of great literature and great music, provides help and counsel to young and old when they are in trouble, educates children in moral responsibility, and worships God in its own fashion.
